Program: Outreach Peer Support
Organization: Northern Initiative For Social Action
Description of Services: The Outreach Peer Support Program offers a supportive relationship for individuals who are receiving services from Canadian Mental Health Association Sudbury/Manitoulin’s housing programs. Support may be provided in the form of: one-to-one interactions; group activities that promote creative expression, physical wellness and/or cultural connections; assisting individuals with activities of daily living such as grocery shopping, meal planning or organizing; supporting individuals during medical and community appointments; and connecting individuals to other community services and resources.
Requirements
Age: 16 years and older
Application: Referral by Canadian Mental Health Association (CMHA) staff for any recipients of housing services from CMHA (Fairview, Moonlight). Referrals may be initiated by the individual if they are involved with CMHA housing services.
