What is a referral?

A referral from a client, or from one health service provider to another, is a "request to assess" for services. The assessment confirms the need, eligibility, and availability of the service. The referral contains the key information needed for the agency receiving the referral to contact the person being referred.

Can I refer myself to services?

Anyone can use the referral forms, though each organization has its own referral requirements. Please check the "Application" field in the service profile you are interested in.

If I can complete the referral form, how do I send it in?

All referral forms should be faxed or hand-delivered. If you do not have access to a fax machine, most public libraries provide this access.

Fax is considered the most secure form of electronic communication. Email is not a secure method of transferring personal health information. Some health service providers have policies that emails cannot be used to transmit any patient or client information. If you have questions about the referral process for a particular agency, please contact them directly by phone.

What do I do if I don't hear back from the agency I've contacted?

What do I do if there isn't a service available in my area?
