Program: Community Aftercare
Organization: Gwekwaadziwin Miikan Mental Health And Addictions
Description of Services: A gradual transition to either independent living, or living with their family, and full reintegration into their community. Participants will be offered appropriate referrals to community services and peer support groups. At the same time, staff will maintain contact and offer support via meetings, phone, text message and social media.

Fees: Yes, Individuals in this program are required to pay a geared to income program fee, and be responsible for all of there own groceries, transportation, personal care costs

Application: Referrals are accepted from those who have completed treatment at this organization.
