Program: Positive Steps Case Management Program

Description of Services: The program serves individuals who have been diagnosed with a severe mental illness such as schizophrenia, schizoaffective disorder or other psychotic disorders; and individuals who have multiple and complex needs. The program consists of: case management, Community Treatment Order Planning (CTO), administration of long acting medication, and rehabilitation and recovery programs.
Requirements
Eligibility - Population(s) Served: Community Treatment Order Only
Age: 16 years and older
Application: The program accepts self-referrals, as well as those initiated by health professionals, community organizations and family members. Individuals must have an attending psychiatrist.
