Program: Housing Case Management - Sudbury - Elm Street

Description of Services: The Housing Case Management program offers time-limited support to individuals with transitioning to a new living environment or maintaining their current housing-with the goal of successfully living independently. A Housing Case Manager provides one-on-one emotional and practical support in developing life skills and coping strategies for leading a purposeful life. A Housing Case Manager may also support with navigating community services to further support individuals with achieving their housing-related goals.
Requirements
Age: 16 years and older
Application: Self-referrals accepted. Call, walk-in or complete the on-line form to initiate contact and arrange for an intake. An intake assessment includes a discussion of opportunities, desired outcome and possible referrals to supports and services provided internally or by other community organizations. The intake worker is also available to assist with issues that require brief service and to provide general information and assistance to family members.
