Description of Services: |
The Supportive Independent Living program refers to a variety of services offered to persons affected by a severe and persistent psychiatric illness.
These services are aimed at increasing individuals' well-being by offering clinical follow-up services, advocacy, coordination of services, family support, rehabilitation and the management of medical needs. Psychiatric consultation available on-site. |

Application: | Referrals may come from institutions and other health organizations that can provide sufficient information to indicate that the person has been diagnosed by a psychiatrist (ex. psychiatric report). French to follow: Les références au programme de soutien à la vie autonome sont internes. Les références peuvent provenir d'instituts et d'autres agences de santé pouvant fournir de l'information suffisante indiquant que la personne a été évaluée par un psychiatre (ex. rapport psychiatrique) et qu'un diagnostic de maladie mentale sévère et persistante a été établi. | ||
