Eligibility - Population(s) Served: | Residents of the City of Greater Sudbury 12 years of age or older requiring increased symptom management provoking hospitalizations, emergency room visits, physician visits, etc. * life expectancy of 12 months or less * non cancer illness or client with a cancer diagnosis no longer able to attend Northeast Cancer Centre Symptom Management Clinic (SMC) and can only be seen at home | ||
