Description of Services: Support services for people with long term health care needs

Respite programs - Provides respite to caregivers who required support so they may be away from the home overnight

Caregiver Support Program - Helps to maintain the well-being of a caregiver caring for a loved one * 24 hour Seniors Attendant Care in-home helps with medications, meal preparation, personal care, ROM exercise, and foot care

PATH program - Assists older adults in transition from hospital to home where a PSW worker will assist the older adults to settle in at home and to obtain food or medications necessary to make the transition to home possible

Adult Day Away Program - Regular social programming for seniors * cost $15/day plus transportation if required * meal, exercise, games, education, caregiver relief
