Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups available on and off reserves Community-led health teams develop programs to improve the social supports and conditions that affect long-term health * culturally appropriate programs and services for Indigenous people, combining traditional healing and Western medical practices Health Promotion - Illness prevention/screening, disease management, pre/post natal health assessments, immunizations, women and men's wellness services, and primary health care discussions Home and Community Care - Health services provided in a home or community setting for older adults, persons with disabilities, or injured persons * includes rehabilitation, hospital discharge services, occupational therapy, and physiotherapy Therapy Services - Occupational therapy and physiotherapy services to provide rehabilitation services to the older adults and disabled members of the community Additional Services:
