Program: Transitional Case Management - Forensic - North Bay - 194 Main Street W
Organization: Canadian Mental Health Association North Bay And District
Description of Services: Community and hospital-based case management for people with serious mental illness and substance use issues who have been found not criminally responsible and are on Ontario Review Board Orders. TCM works in partnership with the Forensic Outreach Rehabilitation Team (FORT) to provide discharge planning and case management services to individuals transitioning from the Forensic Program into the community.
Requirements
Eligibility - Population(s) Served: Forensic Mental Health Only
Age: 18 years and older
Application: Must have primary diagnosis of serious mental illness and be on an Ontario Review Board order or taking up a secure forensic bed in hospital. Referrals through hospital social worker or treatment team member.
