Description of Services: Cedar Street residence offers transitional housing to assist individuals transitioning from hospital or other institution. The goal is to assist people to move to independent living. Residential support workers and case managers work in the home to support individuals towards independent living.
Requirements
Age: 16 years and older
Application: Self-referrals or community organization referrals are accepted. Call during operational hours to complete a referral.
