Description of Services:	For older adults 65 and older living with dementia, cognitive impairments, persons who are disabled, frail as well as older adults who are socially isolated: social integration, physical, recreational and social activities for clients friendly and secure environment professional caring staff and volunteers nutritious meals and snacks transportation may be provided, at an additional cost, if required * transportation to and from the program may be provided within catchment area, and depending upon space availability and client's availability to safely manage vehicle entry and exit
