| Description of Services: | Provides community support as part of a variety of housing models. The program supports tenants to live independently. Case managers assist individuals in setting personal recovery goals and support clients in achieving those goals through connection to outside resources and services. | ||

| Application: | Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. | ||
