Description of Services: Provides high-support housing vulnerable, at-risk and frail older adults and seniors in the community. The program offers 24-hour on-site staffing and access to services for seniors with serious mental illness. Program assists residents health and medical concerns, mental illness, and substance use problems and other related issues. Personal Support Workers provide practical assistance and care to residents, including personal care, housekeeping, laundry and escorts to appointments, enabling them to remain in the community as they age.

Application: Referrals go through The Stepping Stone Project. Please visit the website for referral forms and specific eligibility criteria. Referrals are accepted from professionals.
