Program: Community Outreach And Support Team (COAST) - Assertive Community Treatment Teams (ACTT) - Parry Sound

Description of Services: The function of COAST is to assist people in moving towards recovery and to facilitate a higher level of independence in the community. The involvement of family, friends and other supports is encouraged and welcomed. COAST is a mobile service that provides comprehensive and individualized treatment, housing, medication, and rehabilitation (e.g. employment, finances, cooking etc.). The intensity and duration of service is based on each person's needs and wishes. COAST provides services throughout the entire Parry Sound and Muskoka regions with teams in Parry Sound, Sundridge, and Bracebridge.
Requirements
Age: 16 years and older
Application: Individuals can be referred to COAST through the intake service. Clients must meet specific ACTT admission requirements. Exclusionary Criteria includes possible primary diagnosis of personality disorder, present issues of unpredictable violence or assaultive behaviour, diagnosis of acquired brain injury, or a person needing short term intensive support to achieve goals in one life domain.
