Description of Services: Important characteristics of the program are: - serves individuals with serious mental illness that are complex and have devastating effects on functioning - services are delivered by a group of multidisciplinary mental health staff who work as a team to provide the majority of the treatment, rehabilitation, and support services clients need to achieve their goals. - services are individually tailored with each individual to address the preferences and identified goals - delivers services in community locations to enable each client to find and live in their own residence and find and maintain work in community jobs ACT services are delivered in an ongoing rather than a time-limited framework to aid the process of recovery and ensure continuity of a caregiver.
Requirements
Age: 18 years and older
Application: Self-referrals accepted. A referral may be initiated by a family or service provider. Call or walk-in during operational hours to complete a referral. Must meet Assertive Community Treatment (ACT) admission criteria as per Ontario standards.
